California Casualty Management Company (”Sponsor”) is offering the Online Sweepstakes (”Sweepstakes”). Sponsor, in its sole discretion, may alter, amend, extend, modify or terminate this contest at any time. NO QUOTE OR PURCHASE NECESSARY. A PURCHASE DOES NOT IMPROVE YOUR CHANCE OF WINNING. VOID WHERE PROHIBITED. Entrant must be 18 and a resident of the fifty (50) United States and the District of Columbia. Residents of AK, HI, MA, MI, NC, NY, RI and WI are NOT eligible. Entrant must be currently employed as or retired as a Nurse (RN, LPN or NP), a member of the Ohio Nurses Association (ONA), or a member of the Oregon Nurses Association (ONA)

Employees of Sponsor, the Sponsor’s parents, subsidiaries, affiliated companies, and agents and the immediate family (defined as parents, spouse, children, siblings, grandparents) of each such employees, and all those with whom such employees are domiciled, are NOT eligible.

Unless otherwise noted, a contestant may only submit one entry. The collection and use of entrant information will be governed by Sponsor’s online privacy policy, available for viewing at http://www.calcas.com/web/ccmc/privacy.

Entries must be received by Sponsor by the specified drawing date to be eligible for the indicated drawing. There is 1 (one) drawing in a calendar year.



Each entrant selected as a potential winner must comply with all terms and conditions set forth in these Official Rules, and winning is contingent upon fulfilling all such requirements. For the prize, a potential winner will be selected in a random drawing from all entries received on or before the entry deadline. Sponsor will be solely responsible for selecting the recipient, interpreting contest rules, and all other matters related to the contest. Its decisions will be final.

Odds of winning depend on the number of eligible entries received. Potential winners will be notified by e-mail or mail as close as possible to the timeline proposed in the drawing calendar. If a potential winner cannot be contacted within fourteen (14) days after the first attempt to contact such potential winner, an alternate entrant will be selected in his or her place at random from all entries received.

The prize will be delivered in the form of a check in the amount of $1,000 made payable to the recipient designated on the winning entry. Prize is nontransferable and must be accepted as awarded. Winner is responsible for all taxes on the value of the prize, including federal, state and local income taxes. Sponsor is not responsible for lost or mutilated prizes and none will be replaced. No substitution may be made, except at Sponsor’s discretion. In which case, Sponsor will provide a prize of equal value.

Potential winners will be required to sign and return an Affidavit of Eligibility, Liability Release, and where lawful, a Publicity Release, within fourteen (14) days of notification, failing which the potential winner will be disqualified and another winner selected in the place of the disqualified person at random from all entries received. By accepting the prize, the winner agrees that the Sponsor and those acting under Sponsor’s authority, may use winner’s name, picture/portrait likeness and/or voice, for advertising and promotional purposes without further consideration, unless prohibited by law.

BY ACCEPTING THE PRIZE, THE WINNER AGREES THAT THE SPONSOR(S), THEIR SUBSIDIARIES, AFFILIATES, ADVERTISING AND PROMOTION AGENCIES, AND ALL OF THE SPONSOR’S OFFICERS, DIRECTORS, EMPLOYEES, REPRESENTATIVES AND AGENTS, WILL HAVE NO LIABILITY WHATSOEVER FOR, AND WILL BE HELD HARMLESS BY WINNER FOR ANY LIABILITY FOR ANY
